
Testicular Ascent and Scrotal Support During Intercourse

The testicles rise toward the body during sexual arousal. This is normal, and most men do not notice it. For some it is uncomfortable, and the discomfort competes with sensation during intercourse. This article explains the reflex responsible, when it becomes a problem, and how the anchoring loop of a constriction device affects it.
The cremasteric reflex
Each testicle is surrounded by the cremaster muscle. When it contracts, the testicle is drawn upward toward the inguinal canal. This is the cremasteric reflex.
The reflex serves two purposes. It regulates temperature, drawing the testicles closer to the warmer body when cooling is not required. And it is protective, contracting in response to cold, stress, touch to the inner thigh, and physical activity.
The testicles normally rise during arousal and remain elevated approaching ejaculation. This is a standard part of the sexual response cycle.
When it becomes uncomfortable
For most men this movement is unremarkable. It can be uncomfortable when the reflex is unusually strong, when the cremaster muscle or surrounding structures are sensitive, or where there is an underlying condition affecting the scrotum.
Men describe a dragging, aching, or pulling sensation. Elevated testicles can also be compressed between the pubic bone and a partner's body in some positions, which produces a distinct pinching discomfort.
Discomfort of this kind is a distraction rather than a danger, but it can lead men to avoid certain positions or shorten intercourse.
How the anchoring loop affects it
Xialla's loop passes around the scrotum, and the band draws it rearward and holds it against the body. The primary purpose of this is to keep the ring in position so that venous occlusion is maintained during movement. A secondary consequence is that the scrotum is held in a lower, more stable position, which limits how far the testicles can be drawn up.
The support is light. The loop holds position rather than applying compression, and it does not prevent the reflex, only the extent of the resulting movement.

Important: persistent scrotal pain needs assessment
This article concerns ordinary discomfort from testicular ascent. It does not concern scrotal pain, which is different and should be evaluated.
Persistent or recurrent scrotal pain can indicate varicocele, epididymitis, a hydrocele, a spermatic cord problem, or, rarely, testicular cancer. Sudden severe scrotal pain can indicate testicular torsion, which is a surgical emergency requiring immediate medical attention.
Chronic scrotal pain is associated with reduced sexual function and interest, and it is treatable. If you have ongoing scrotal pain, see a physician rather than managing it with a support device. Do not use a device that applies rearward traction to the scrotum if you have unexplained scrotal pain, a known varicocele, or a recent scrotal or inguinal procedure without discussing it with your doctor first.
What the evidence supports
The cremasteric reflex and its role in the sexual response cycle are well established. That a loop holding the scrotum rearward limits testicular ascent follows mechanically from the design.
Whether this improves comfort or satisfaction during intercourse has not been studied. Xialla's published evidence addresses erectile function and blood retention. Men who use the device report the effect, which is a reasonable basis for describing it but not for claiming it as a demonstrated outcome.
Safe use
Observe a maximum of 30 minutes of continuous wear, remove the device before sleeping, and remove it immediately if numbness, coldness, discolouration, or pain occurs, including scrotal pain. Seek immediate medical care for sudden severe scrotal pain, which can indicate testicular torsion.
The loop should feel supportive rather than tight. Traction sufficient to cause aching or dragging in the scrotum is too much, and the device is not intended to hold the scrotum under tension.
